Full refunds are only issued for cancelled classes before class starts, otherwise refunds are prorated. The instructor will make every effort to notify students of postponements and/ or cancellations at least 24 hours before the first scheduled class. Also, we understand that emergencies come about. An eemergency (ex. Illness, accident, etc.) will be excused, but if you say you are coming and last minute don’t or for no shows after the 1st incidence the student will be responsible for the amount due for class. If something comes up please notify instructor of absence and if on instructor’s part you will be notified as soon as possible. Dates and times are subject to change as necessary. If you wish to withdraw from a class and receive a refund, you must notify the instructor at least two (2) business days before the first class meeting. This constitutes a commitment to taking these classes and the expectation is that all scheduled classes will be attended.PARENT OR GUARDIAN: PLEASE READ, THEN SIGN AND DATE BOTTOM LINEI certify that the above named enrollee has no condition that prohibits full participation in acting classes registered for. I release the instructor Cecilia Wheeler, volunteers, and affiliates from all liability in connection to any personal injury and/or damage to or loss of personal property while engaged in class activity or commuting/walking to or from class.Medical EmergenciesI grant the staff permission to engage in first aid (if trained) and if necessary to transport or have myself or my child transported to the nearest Emergency Medical Facility. Fees, Reimbursements and Refunds1. I and/or my child agrees to exercise care in the class, and I agree to reimburse the instructor for any personal breakage, damage or loss of property due to my/his/her negligence. 2. It is the student or student’s family responsibility to cover a $25 returned check fee plus the original check amount resulting from a check which has been returned by the bank. 3.
